The Endometriosis Team
Effective and complete treatment of Endometriosis is not a one-person job. It requires a team of experts working together. The medical jargon is a multidisciplinary team. That simply means a team of specialists from different specialties, working together for a common goal.
The patient is always at the centre. After all, everything we do is for the patient.
Putting together a team of expert surgeons who are passionate about endometriosis is not an easy task. The team should read from the same hymn sheet, share the philosophy of care and trust each other and work as one.
The pictogram below shows the different specialists required for successful treatment of advanced endometriosis.
The Endometriosis specialist nurse (Clinical Nurse Specialist or CNS) has a crucial role. She is the first point of contact with the patient, before and after the operation, making sure all their questions are answered. They help coordinate the whole care pathway, organise the multidisciplinary team meetings and ensure the patient is fully prepared for surgery.
Colorectal Surgeons (or bowel surgeons) specialise in complex procedures on the bowel. They get involved, usually when endometriosis has invaded into the muscle of the bowel and part of the bowel needs to be removed (segmental bowel resection). Mr. Khazali has worked closely with the same team of very experienced and skills colorectal surgeons over the years.
Urologists get involved when the ureters (tubes that take urine from the kidneys to the bladder) are significantly affected or invaded by endometriosis. Kidney Transplant surgeons are very experienced in re-implanting ureters into bladders when this needs to be performed because of ureteric involvement that is beyond repair.
When endometriosis involves the diaphragm or the lungs, working together and consulting a cardiothoracic surgeon would usually be necessary.
A neurosurgeon may be needed to join the multidisciplinary team in special circumstances where specific parts of big pelvic nerves need to be operated on.
Other specialists such as Radiologists, pain specialist and fertility experts play a very important role in the multidisciplinary team.
Mr. Shaheen Khazali

Mr Khazali is an internationally renowned expert in advanced endometriosis surgery. His practice, both on the NHS and privately, is almost exclusively dedicated to looking after women with the most severe forms of endometriosis when the disease has affected other organs such as the bowel, bladder, ureters and pelvic nerves.
He is one of very few surgeons worldwide who has formal training in Neuropelveology, which enables him to offer treatments to those patients where pelvic nerves are affected either by endometriosis or other pathologies. He is currently the honorary secretary of the International Society of Neuropelveology (ISON).
He also sees and treats women with other benign gynaecological conditions such as heavy menstrual bleeding, ovarian cysts and menstrual disorders.
He is also an NHS Consultant at Ashford and St Peter’s Hospitals NHS Foundation Trust (ASPH) in Surrey. ASPH has a long history in treatment of severe endometriosis for many years and was one of the first few centres accredited for treatment of sever endometriosis by the British Society for Gynaecological Endoscopy (BSGE). In 2012 Mr Khazali co-founded CEMIG (Centre for Endometriosis and Minimally Invasive Gynaecology) at ASPH, which has been one of the busiest accredited centres for treatment of severe endometriosis in the UK. In 2018 and again in 2019, CEMIG treated more patients than any other accredited endometriosis centre in the UK, making it the busiest BSGE centre
Mr Khazali also travels to Iran regularly where he teaches advanced endometriosis surgery.
Mr Khazali has led a number of innovative projects, including an online pain diary for chronic pelvic pain patients, development of a new laparoscopic instrument to improve safety and speed of surgery and also has devised a scoring system for assessing laparoscopic surgical skills of trainees. He is an elected member of the council for the BSGE and is the editor of BSGE’s video library and forum, which he has helped set up.
Mr Khazali has published in various journals, contributed to a number of book chapters in the field and regularly presents at national and international meetings.

Mr Pasha Nisar FRCS, is the Specialty Lead and Consultant General and Colorectal Surgeon at Ashford & St Peter’s NHS Trust, Chertsey. He is also the Lead Clinician for colorectal cancer and inflammatory bowel disease surgery. An expert in keyhole surgery, he performed the Trust’s first single incision ‘scarless’ operation for colon cancer and the first artificial sphincter implant. He leads the development of the Trust’s robotic surgery programme.
Mr Nisar is the Lead Consultant for Endoscopy at Princess Margaret Hospital, Windsor and Founder of the Colorectal Research Programme at Royal Holloway, University of London, where he supervises PhD studies on colon cancer genetic biomarkers.
Mr Nisar grew up in Sunningdale and graduated in medicine from Trinity College, Cambridge University in 1998. He was awarded a Doctorate from The University of Nottingham for his thesis in haemorrhoids and pelvic floor surgery at the Academic Surgical Unit.
Mr Nisar’s specialist surgical training in endoscopy, laparoscopic, colorectal and hernia surgery was completed in the London Deanery – including St George’s, Frimley Park, Royal Surrey Hospitals. He then undertook a colorectal surgery fellowship for one year at the internationally renowned Digestive Disease Institute, Cleveland Clinic USA. Mr Nisar is a high volume laparoscopic surgeon with in depth experience encompassing all aspects of colorectal disease, colorectal surgery and hernia repair. He is an expert in minimally invasive surgery and enhanced recovery.

Mr Phil Bearn has been a Consultant General and Colorectal Surgeon in the Department of Surgery at Ashford & St. Peter’s Hospitals NHS Trust since 1999 and honorary senior lecturer at Royal Holloway, University of London since 2014. From 1995 to 1999, he was a consultant at the Good Hope Hospital in Birmingham.
He received pre-clinical training at Cambridge University and clinical training at Guys’ Hospital. His senior training included rotations at Guys, Charing Cross, Hammersmith and UCH. He was awarded a MS by London University for a study involving mesothelial cells. During this period he was awarded a junior BHF fellowship. Post research, his senior training included placements in world renowned units such as Basingstoke under Bill Heald and the Anorectal Physiology Unit at St Mark’s under Mike Henry.
As a consultant he founded anorectal physiology units in Chertsey and Birmingham. He was responsible for starting laparoscopic colorectal surgery in both units. He has gained a wide experience in the medical and surgical management of colorectal disease including pelvic floor problems, colorectal and anal cancer, inflammatory bowel disease and endoscopy.

Mr Andrea Bille is a Consultant Thoracic Surgeon at Guy’s and St Thomas’ NHS Foundation Trust and Honorary Senior Clinical Lecturer at Kings College University, London.
He specialises in the treatment of thoracic conditions and in surgery and sees patients privately for a range of conditions including Lung cancer, Pneumothorax, Pectus deformity, Infection of the pleura / empyema, Rib fractures, Pleural effusion Thymoma, Mesothelioma and Mass/lesion in the mediastinum.
Mr Bille attended medical school at the University of Turin, Italy. He trained in Cardiothoracic Surgery in Turin and Guy’s & St Thomas’s Hospital, London. He qualified in 2011. Following this he did a clinical fellowship at Guy’s Hospital in mesothelioma surgery and minimally invasive thoracic surgery.
In 2012 he was appointed as a Consultant at the National Institute of Cancer in Milan, focussing on surgical treatment of lung cancer, thoracic sarcoma and mediastinal tumours which require complex reconstruction.
He was awarded a PhD in 2015 following a three-year research programme on surgical treatment of mesothelioma. Pursuing a particular interest in the surgical thoracic oncology, he undertook a clinical fellowship at Memorial Sloan Kettering Cancer Center in 2014/15, the highest ranked oncological centre in the US. He was trained in minimally invasive technique for the treatment of thoracic malignancies and advance technique with complex reconstruction for extensive thoracic malignancies.
In 2015 Mr Bille was appointed Consultant Thoracic Surgeon at Guy’s & St Thomas’ Hospital where he treats benign and malignant thoracic disease. He routinely performs minimally invasive surgery and robotic surgery for lung and mediastinal conditions.
He is involved in teaching of medical students and since 2016 has held an academic role at Kings College University as Honorary Lecturer in the Cancer division, focusing his research on mesothelioma, chest wall reconstruction and lung cancer.
He has published more than 80 scientific articles and has given many presentations at national and international meetings, including the European Thoracic Meeting and the World Conference on Lung Cancer. He is the Associate Editor of the Thoracic section of the Tumori Journal, indexed on PubMed.
He is a member of the staging committee for Lung Cancer (N Subcommittee), Mesothelioma and Thymoma (IASLC Association). This committee creates and validates the TNM (tumour, node, metastasis) staging system used for thoracic malignancies around the world. He is also Chair of the European Registry of Mesothelioma in ESTS (European Society of Thoracic Surgeons), and a member of ITMIG (International Thymic Malignancies Interest group). He has been part of the organising committee of the 20th edition of the World Conference on Lung Cancer hosted in Barcelona in 2019. He has been course director of the Europe involved in research on Thymoma.
